Welcome to Physiocare! Our friendly staff is here to help you get started down the road to recovery. The information below is provided to help you prepare for your treatment here. To schedule an appointment call our clinic between 7 am and 7 pm Monday through Friday.

How to prepare for your first appointment:

  • Bring a prescription or referral from your physician if you have one (most insurance plans require that you do).
  • Bring all related medical records and/or diagnostic reports (X-rays, MRI, etc).
  • Bring a valid insurance card or claim information (motor vehicle or Labor & Industries).
  • Our required paperwork can be downloaded and printed from the files below: Please print these forms, fill them out, and bring them with you to your appointment.
  • Bring or wear loose fitting, comfortable clothing that you can move around in. Active wear such as shorts, sweats, and t-shirts are most appropriate.
  • For orthotic appointments be sure to bring or wear the appropriate footwear and shorts or pants that can be rolled up above your knees.

Insurance:

Understanding your insurance benefits is important and knowing your benefit for physical therapy will help ensure a positive experience. Most insurance plans cover physical therapy at some level, and depending on your plan may require a physician referral. We encourage you to call your insurance company and check on your physical therapy benefit before you come in. However, as a courtesy to you, we can call to check on your benefits for you. PhysioCare is a preferred provider with most insurance plans in the Northwest.

Cancellations:

We value your time and ask that you value ours by giving at least 24 hours notice for any cancellations or changes to your appointment (excluding illness or emergency).
